GLP-1 Weight Loss Drugs and Medical Tourism: Buying Ozempic and Wegovy Abroad

The prices are real, and so are the counterfeits. Here is how Americans source semaglutide and tirzepatide overseas without gambling on a fake.

About one in eight American adults now say they are currently taking a GLP-1 drug, and nearly one in five say they have taken one at some point, according to KFF's 2025 health tracking poll. The results can be striking. In the SURMOUNT-5 trial published in the New England Journal of Medicine in 2025, patients lost 20.2 percent of their body weight over 72 weeks on tirzepatide and 13.7 percent on semaglutide, as summarized by the American College of Cardiology. The catch is the price. A month of Ozempic carries a US list price of about $1,028, and roughly half of GLP-1 users tell KFF the drugs are hard to afford.

That gap between what a medication delivers and what it costs is the same force that sends Americans overseas for dental work and knee replacements. So it was only a matter of time before it reached the pharmacy counter. If you are weighing that decision, this guide walks through what is legal, what is safe, and where the real savings actually sit. It sits inside our larger guide to medical tourism for Americans, and alongside our coverage of bariatric surgery abroad for people considering surgical options.

GLP-1 receptor agonist: a class of medications that copy glucagon-like peptide-1, a gut hormone that helps regulate blood sugar and appetite. Semaglutide (sold as Ozempic and Wegovy) and tirzepatide (sold as Mounjaro and Zepbound, which also acts on a second hormone called GIP) are the best known. They are taken as weekly injections and were approved for type 2 diabetes and obesity.

Why are GLP-1 drugs so much cheaper outside the US?

The United States has no central body that negotiates drug prices, so manufacturers set list prices far above what other wealthy countries pay. The same Ozempic pen that lists near $1,028 a month in the US lists around $83 in France and $147 in Canada, according to the Peterson-KFF Health System Tracker. Across weight-loss and diabetes GLP-1s, Americans pay several times what patients in peer nations pay for the identical product.

The pattern holds for the weight-loss branded version too. Wegovy lists near $1,349 a month in the US, more than four times its roughly $328 list price in Germany, per the same Peterson-KFF analysis. In-person retail prices at pharmacies in countries like Mexico often track well below US levels, though local stock and pricing move week to week.

Is it legal to bring GLP-1 drugs back to the US?

In most cases, yes, within limits. Federal law generally does not permit importing prescription drugs for personal use, but the FDA uses discretion to allow travelers to bring in roughly a 90-day supply of a medication for personal use when it treats a serious condition and does not present an unreasonable risk, as the Congressional Research Service explains in its prescription drug importation brief. GLP-1s are not controlled substances, so the separate 50-unit border cap on controlled drugs does not apply, but the personal-use ceiling and documentation expectations still do.

If you plan to carry medication across the border, line up the paperwork before you fly:

    1. Bring a valid prescription, ideally written in English, that matches the drug and dose.
    2. Keep the product in its original, labeled packaging.
    3. Stay within a 90-day personal supply, since larger quantities can be questioned or held at customs.
    4. Carry it in your hand luggage with a cold pack, because injectable GLP-1s are temperature sensitive.

How do you avoid a counterfeit GLP-1?

Buy only from a licensed pharmacy or an accredited clinic, insist on sealed and correctly labeled product, and protect the cold chain end to end. The counterfeit risk is real and documented. In June 2024 the World Health Organization issued its first global alert on falsified Ozempic after fake batches turned up in Brazil, the United Kingdom, and the United States.

The FDA has its own warnings for anyone tempted by a cheaper unbranded vial. The agency notes that some sellers use salt forms such as semaglutide sodium or semaglutide acetate that are not the FDA-approved molecule and have not been tested for human use, and that counterfeit or improperly handled product can carry the wrong dose or contamination. Its overview of the risks of unapproved GLP-1 drugs is worth reading before you buy anything you cannot verify. A shipment that arrives warm, or a pen with a batch number the manufacturer does not recognize, is a reason to walk away.

Should you travel for a GLP-1, or is there a better fit?

Traveling for medication makes the most sense when you already have a reason to go, when you can buy from a verified pharmacy or clinic, and when you have a US clinician managing your dose and refills after you return. The savings are largest on brand-name product bought at a peer-nation list price, and smallest once you factor in flights and the cost of coming back every few months for a new supply.

For some people, a different route fits better. If your goal is durable weight loss and you carry significant excess weight, surgery may deliver more per dollar over time. Our comparison of gastric sleeve cost by country lays out those numbers, and our profile of bariatric surgery in Tijuana covers the destination Americans reach most easily. The table below is a starting point for thinking about the trade-offs rather than a prescription.

Route Prescription needed FDA-approved product Main thing to check
Buy abroad in person Yes Yes, if branded from a licensed pharmacy Seller verification and cold chain
Compounded or online seller Sometimes Often no Whether the molecule and pharmacy are legitimate
US brand through insurance Yes Yes Coverage and prior authorization
    Source FDA guidance, 2024

Whatever you decide, the safest version of this involves a real clinician, a verifiable supply, and continuity of care once you are home.
