Brazilian Butt Lift Cost in the Dominican Republic: What the Price Leaves Out
Santo Domingo packages look like a fraction of a US fee. Here's what they usually cover, what gets added, how to check your surgeon, and what the CDC has found.
The American Society of Plastic Surgeons put the US surgeon's fee for a Brazilian butt lift at $7,000 to $11,500 in its 2024 cost report, and that's before anesthesia or the operating room. Santo Domingo packages advertise a whole BBL for less than that fee alone. So it's no surprise the Dominican Republic sits near the top of many people's lists. But the sticker price is only one line of what you'll actually spend, and the CDC has counted 93 Americans who died after cosmetic surgery there between 2009 and 2022. You deserve to see both numbers before you put down a deposit.
This guide covers what a Dominican BBL quote usually covers, what tends to be priced separately, how to check who'll be operating, and what to have in place for when you're back home.
How much does a Brazilian butt lift cost in the Dominican Republic?
Package prices published by Santo Domingo clinics and booking sites we reviewed in October 2026 mostly ran from about $2,500 to $6,000 for a standard BBL, with combined surgeries such as a BBL plus tummy tuck quoted higher. No Dominican government body or surgical society publishes prices, so those figures are advertised starting points, set by the clinics themselves.
The US comparison needs care, because the two numbers don't measure the same thing. ASPS counts only the surgeon's fee, and its 2023 report gave a single national average of $7,264 for buttock augmentation with fat grafting, the clinical name for a BBL. A Dominican package usually bundles the surgeon's fee with anesthesia and the operating room, and some add a compression garment or a few nights of recovery housing too. That's why the gap looks so wide on paper.
What is a Brazilian butt lift?
A Brazilian butt lift is a fat transfer. The surgeon removes fat by liposuction, usually from the belly and flanks, then injects it into the buttocks to add shape and volume. No implant is involved. Because the fat has to come from somewhere, the liposuction is part of the operation, and how many areas are treated is one of the biggest drivers of the final price.
| What you're comparing | Typical figure | What it includes | Source |
|---|---|---|---|
| US surgeon's fee, BBL | $7,000–$11,500 | Surgeon only; anesthesia, facility and other costs extra | ASPS cost report, 2024 |
| US surgeon's fee, BBL (single average) | $7,264 | Surgeon only | ASPS cost report, 2023 |
| Dominican BBL package, advertised | about $2,500–$6,000 | Varies by clinic; often surgeon, anesthesia and operating room | Clinic and booking-site listings reviewed by JetWell, October 2026 |
| US health-system cost of treating problems after cosmetic surgery abroad | about $12,760 per patient on average | Hospital and surgical care at one US health system | Hery et al., Aesthetic Surgery Journal Open Forum, 2023 |
That last row comes from a 2023 study of 41 adults seen at one US health system within a year of surgery abroad. Their care added up to $523,272, which works out to roughly $12,760 each, and the procedures they'd had most often included tummy tucks, liposuction, breast augmentation and BBLs. It's a small study from one place, so it can't tell you your own odds. But it's a useful reminder that the lowest quote isn't the whole bill if something goes wrong.
Are there any add-ons once I'm there?
Often, yes, and they're far easier to sort out before you fly than after. A BBL quote is built on assumptions about how many areas get liposuction and what you'll need around the surgery itself, and anything outside those assumptions can be priced separately once you've arrived. Ask for every one of these in writing before you book:
| Item | Ask the clinic |
|---|---|
| Liposuction areas | How many areas does the price cover, and what does each extra area cost? |
| Pre-op blood work and tests | Are labs, an EKG or a clearance visit included, and what happens to my money if a test result means surgery can't go ahead? |
| Anesthesia and operating room | Are both inside the package, and for how many hours? |
| Compression garment (faja) | Is the first garment included, and what does a second-stage garment cost? |
| Post-op massages | How many lymphatic massage sessions are included, if any? |
| Recovery house or hotel | How many nights, who's on staff overnight, and is transport to check-ups included? |
| Medications | Are pain medication, antibiotics and blood thinners included? |
With JetWell, everything on your Care Plan is price-locked, and anything that isn't on it is extra. That's the same rule you should hold any quote to, wherever it comes from. Read the deposit terms before you pay, too. A JetWell deposit goes toward your package, and it's worth knowing exactly what any clinic's deposit does and doesn't cover before you send it.
Who actually does my BBL?
Your BBL should be done by a plastic surgeon who holds a Dominican exequátur and belongs to Sodocipre, the Dominican plastic surgery society, and you can check both before you book. Get the surgeon's full name in writing, look it up on Sodocipre's published member list, and ask for the exequátur number. If the clinic won't name the surgeon, treat that as your answer.
What is an exequátur?
An exequátur is the government authorization a doctor needs to practice medicine in the Dominican Republic, granted by the Ministry of Public Health. Sodocipre only admits doctors who hold one and have trained in general and plastic surgery, according to El Caribe, which is why its member list is a good first check.
The CDC's 2024 report on Americans who died after cosmetic surgery in the Dominican Republic shows why these questions matter. Annual deaths rose from an average of 4.1 a year in 2009–2018 to 13.0 a year in 2019–2022. Of the 24 women whose medical records investigators could review from 2019–2020, every one had liposuction and 92% had fat moved to the buttocks. Their average BMI was 32, and the CDC named obesity and having several procedures in one operation as common risk factors.
A pulmonary fat embolism happens when fat gets into a vein and travels to the lungs, where it blocks blood flow. In the 20 deaths from 2019–2020 with an autopsy report, the CDC attributed 11 to fat embolism and seven to blood clots in the lungs. That pattern matches what surgeons have found worldwide. In the 2017 ASERF task force report, 692 surgeons reporting on 198,857 BBLs counted 32 deaths from fat embolism, and surgeons who injected into the deep muscle saw significantly more of them. A 2022 practice advisory in Aesthetic Surgery Journal added ultrasound guidance to check where the cannula sits during injection, and a cap of three BBLs per surgeon per day.
So once you've got the surgeon's name, ask whether fat goes only under the skin, above the muscle, and whether ultrasound is used to confirm it. Ask how many BBLs that surgeon does in a day as well, and if anything else is planned for the same operation.
What happens if something feels wrong after I'm home?
Call the clinic and a US doctor the same day, and go straight to an emergency room for chest pain or sudden trouble breathing. Problems can surface weeks after you've flown home. In the 2023 US study above, seromas (fluid pockets) and infections were the most common reasons people needed care, and a CDC investigation published in Emerging Infectious Diseases in 2016 traced slow-growing mycobacterial infections in 21 patients across six states to five Dominican clinics. All nine patients from the main clinic with full records needed further surgery.

The CDC Yellow Book warns that flying after surgery raises the risk of blood clots, and suggests seeing a travel medicine provider at least four to six weeks before you go. Our guide to flying after cosmetic surgery covers timing in more detail. Before you leave Santo Domingo, get your operative report and medication list in English, and line up a US doctor who's agreed to see you. If you don't have one yet, here's how to find a US doctor for follow-up.
What do I need to travel to the Dominican Republic for surgery?
US citizens can stay up to 30 days without a visa, but the State Department says your passport must be valid for six months from arrival, and you'll fill in the country's eTicket form for both entry and exit. It also notes that most private hospitals expect payment up front and don't take US insurance, so check what your own plan and any travel cover will and won't pay for. Our guide to insurance for care abroad explains the options.
